To provide you with the highest quality of personalized, uncompromised care, our clinic operates as a cash-pay (out-of-pocket) practice at this time. We do not accept insurance directly. However, we can provide you with a detailed receipt (superbill) upon request, which you may submit to your insurance provider for potential out-of-network reimbursement.
We do have the option for a sliding scale cash pay service. Sliding scales are on a case-by-case basis and at the discretion of the practice. Feel free to bring this up in your consultation and we can discuss payment options.

During a new patient visit Dr. Eric will spend time going over a detailed history and together you will create a plan of action that makes sense for your situation. We understand that life happens and things unexpectedly can occur. However, because this time is specifically reserved for you and in ensuring fair access to everyone, we require a 24-hour notice if you need to reschedule or cancel your appointment.
If you do not cancel or reschedule before the 24-hour time period, there is a fee of $100. Recurrent incidences will result in the full amount of the scheduled appointment being charged.

Every individual has different needs and goals, however as a general guide:
If we are just meeting or starting a new treatment, we should plan to meet every 2-4 weeks during this period.
If things are stable, we can plan to meet every 1-3 months.
My policy is to at minimum see my patients every 3 months if we’re working on a medication regimen, and every 6 months if we’re you’re not on medication.
Please note, each time interval can be tailored to one’s needs with more frequent or less frequent visits.
